I'm providing a free downloadable PDF of a preliminary version of the Second Edition of a book. Why? Because I might not live long enough to finish the manuscript and I want the most recent version of it in the public domain. I'm dealing with a medical problem that might not have resolution. It may take another month to complete the book, so until then I'll upload "almost final" versions.
